Web Designer Cancellation of Services Agreement

Website Designers: you can use this form if the Client—not You/Your Company—unilaterally or you both mutually agree to end the Agreement for whatever reason.  It is important to ensure the termination is properly documented to ensure the other party cannot come back later to bring a claim. To complete, you will want to review the “Cancellation” Section of your services contract to see if it affects the below agreement at all.

How to Use this Form:

1) Download the Word Document (so that document can be edited).

2) Read through the ENTIRE AGREEMENT at least once before editing.

    (Become familiar with the form and any documents you may need).

3) Revise the HIGHLIGHTED Information as needed.

4) Read and Delete any NOTES that may be written in.

5) And last, confirm that ALL parties identified. Execute (Sign) the Agreement.
