Photographer – you can use this form to get permission to publish someone’s photograph who is over 18 years of age. This is known in some contexts as a liability waiver—a legal release—that grants you, the photographer, permission to publish the photograph in one form or another for commercial purposes.
1) Download the Word Document (so that you can edit it);
2) Read through the ENTIRE FORM at least once before editing (Get a feel for what it is and any documents you may need);
3) Revise the HIGHLIGHTED Information; and
5) Last, make sure you have ALL parties identified execute (Sign) the Agreement. Keep a COPY of the Signed Agreement in Your files.
